A one row per attempt log for tracking who was contacted about a due checkup, on which channel, what happened, and what comes next.

If more than one person touches your recall list, the log is what keeps them from calling the same patient twice or forgetting a reply that came in on Tuesday. This worksheet gives you one row for every contact attempt. Record the patient, the date they are due, the tier they fall into, the channel you used, the outcome, and the specific next step with a date. When the next step happens, add a new row rather than editing the old one, so the history stays intact.

Use a patient ID or chart number instead of a name if the sheet ever leaves your practice system, and keep clinical details out of the outcome column entirely. The example rows below show the kinds of entries that are useful; replace them with your own. A log like this is also the raw material for your monthly numbers, because counting reached, booked, and kept from these rows is far easier than reconstructing it from memory. If you find yourself filling pages of this every week, that is usually the sign it is time to automate the sending and logging.
