Manual recall outreach is one of those jobs that never shows up on a schedule but eats a surprising share of a front desk week. Someone pulls the list of patients coming due, works through it between check-ins, leaves voicemails, sends texts by hand and tries again a week later. Because it happens in fragments, most practices have no idea how many hours it really takes or what it costs in wages.

This calculator starts from your active patient count and recall interval to work out how many patients come due each month, multiplies by the number of contact attempts each one gets and the minutes each attempt takes, and converts that to staff hours and payroll cost. You then set what share of those touches an automated reminder system would handle, and the tool shows hours and dollars saved. Every rate is an input, so you can be as optimistic or as cautious as your situation deserves.

How to use this tool

  1. Enter your active patient count and recall interval so the tool can estimate how many patients come due each month.
  2. Set how many contact attempts each due patient gets and how long a typical attempt takes, then enter the fully loaded hourly cost of the staff member doing the work.
  3. Adjust the automation share to reflect how much of the outreach a reminder system would take over, then compare the manual hours with the hours and payroll saved.

What the math assumes

  • Patients coming due each month equals active patients divided by the recall interval in months, which assumes due dates are spread evenly across the year.
  • Every due patient receives the full number of contact attempts you enter, even though some book on the first attempt; this slightly overstates manual time.
  • Each attempt takes the same number of minutes regardless of channel, so set the minutes to a blended average of your calls, texts and emails.
  • Hours saved equal fully manual hours multiplied by the automation share you enter; the tool does not assume any particular automation rate.
  • Labor cost saved equals hours saved multiplied by your hourly cost and by twelve months, and it ignores time still spent replying to patients who respond.

Frequently asked questions

How many contact attempts per patient should I enter?

Count what your team actually does today, not what the policy says. If the front desk usually leaves one voicemail, sends one text and tries a second call a week later, that is three attempts.

Is 80 percent automation a realistic default?

It is a starting point, not a measurement. Automated reminders can send the first and repeat attempts on their own, but someone still handles replies, reschedules and patients who need a real conversation, so lower the share if your patient base leans toward phone calls.

Does the saved time turn into real dollars?

Only if you reassign or reduce the hours. For many practices the value is less about payroll and more about the front desk finally reaching every due patient instead of the ones they got to before the phone rang.
